Adult Day Program
The Genesis Program will provide Adult Day Programs and Supports for participants as specified in their Individual Support Plan (written description of services, supports and activities).
The staff of The Genesis Program is committed to serve and provide support to individuals who desire to engage in any activity in a non-residential setting that will encourage social interaction, skill development, responsibility and independence.
Activities that constitute Adult Day Programs include:
Assessment
Personal Care
Skill reinforcement
Training(s)
Recreation and leisure activities
Community service
Assisting the individual with self-medication or provision of medication administration for prescribed medication and assisting the individual with or performing health-related activities
Non-Medical Transportation
The Genesis Program shall provide non-medical transportation for waiver participants solely to access adult day programs, vocational rehabilitation, supported employment-enclave, and/or supported employment- community waiver services as specified in their individual service plan.
Shared Living
The Genesis Program will train and certify Care Companions (caregivers) to assist adults with developmental disabilities in their personal homes. This service includes personal care, supervision, support, housekeeping, community inclusion and other services outlined in the individual service plan.
Supported Living
Supported Living involves services that offers support to individuals with developmental disabilities. The assistance that our agency will provide will insure that each individual served lives in the most convenient and progressive home environment.
The agency will make sure that:
Each individual served will be provided a Direct Support Professional (DSP) to provide personal home care services outlined by the persons served Individual Service Plan (ISP)
We work directly with the individual served and/or the family involved to reach every level of success desired by all parties.
Group Home
The Genesis Program will provide a private residence to a group of adults with developmental disabilities. Each Group Home will be staffed with highly trained Direct Support Professionals to provide personal home care services outlined by the person's individual service plan.
Respite Care
Emergency Respite Program: Services may include, but not be limited to the following circumstances:
Caregiver illness (physical, mental or emotional)
Caregiver hospitalization or doctor appointment
Illness/death of a loved one
Reduction of stress level
Drug/ Alcohol Abuse counseling/support
Care recipient transitions (living arrangements)
Loss of employment/Work related situations
